Applying For Leave?

We want to help make this a smooth transition as you plan your time off. Review below for each step in the process from before you make your request to when you return to work to help guide you through your leave of absence process.
IMPORTANT: Any time employees are missing work for more than three (3) days due to their own serious health condition, to care for a family member's serious health condition, or for a military leave, they are likely eligible for leave through federal and/or state law that provides employees with certain job protections. In these cases, employees are directed to contact FMLASource or Standard to open a request for leave. Please note that employees may be eligible immediately for a qualifying reason.
Before You Apply

Gather the following information to apply for protected leave and paid leave benefits with the Standard and FMLASource.
- Name and social security number.
- Preferred email address.
- Last anticipated day at work.
- Reason for leave and type of leave.
- Reasons: medical, family medical, pregnancy disability, parental bonding, safe leave.
- Types: intermittent, reduced work schedule, or continuous leave.
- Your provider’s name, name of the facility they work at, their phone number, and their fax number.
If FMLASource has permission from you to do so, they can outreach to your provider on your behalf. - The Group Policy Number: 762236
You will need this to apply for paid leave benefits with The Standard.

Making your request

Notify your Supervisor of your leave request.
Let your supervisor know the dates you intend to take leave and that you are making your request.

Open your leave request through FMLASource or Standard at least 30 days in advance of your leave whenever possible.

Complete the Top Off form if applicable.
If applying for Paid Leave Oregon, complete the to use your eligible leave balances to top off your Paid Leave Oregon Benefit. Top off forms received after the 10th of the month may not be processed until the following payroll period. Please review the and the Pay & Benefits while on Leave page for more information about Top Off and pay while on leave.
After your request is open

Complete and provide required documentation.
Complete and provide necessary documentation for your leave to FMLASource and/or The Standard. If you requested leave for your own or a family member's medical condition, work with the provider to complete the required medical certification.

Review communication from FMLASource and/or The Standard.
FMLASource and Standard will send communications via email or U.S. mail to update you on the status of your request. To ensure that there are no delays reviewing your leave request, please review any communication you receive from either vendor.
Please note: The Standard will mail you all communications and paperwork unless you explicitly request to receive them by email.

Report Absences to FMLASource and Standard if you are missing time already.
If your leave has already started be sure to report absences to FMLASource and The Standard. For The Standard, you must report your absences for all leave frequencies (continuous and intermittent both) in order to be paid. Absences may be reported in advance. If you are missing time intermittently, you are required to follow call out procedures and notify your site when you utilize your intermittent leave.
After your leave is approved and returning to work

Continue to report your absences.
Continue to notify FMLASource and The Standard of your absences. For The Standard, you must report your absences for all leaves frequences (conitnuous and intermittent both) in order to be paid. Absences may be reported in advance. If you are missing time intermittently, you are required to follow call out procedures and notify your site when you utilize your intermittent leave.

Confirm your return to work date.
At least two weeks before your planned return to work, confirm your return to work date with: èßäapp¹ÙÍø Leaves Team (leave@pps.net), Your Supervisor, The Standard, and FMLASource.

Prepare to return to work and notify Leaves of any return to work restrictions.
As the end of your leave approaches, check in with your healthcare provider regarding your return to work. IMPORTANT: If your provider sets any restrictions on your return to work, the èßäapp¹ÙÍø Leaves Team may need additional time to coordinate your re-entry into the workplace.
Submit any return to work documentation from your provider to FMLASource, especially if there are any restrictions placed upon your return to work.
