    PTHV TRAINING EVALUATION FORM:  Your input on the PTHV training session is appreciated.  If you did not fill out an evaluation at the training, please use the link below to add your comments.  This information is used to improve PTHV training and to make sure it meets the needs of educators.  Your responses are confidential.  No identifying information is collected on the form.

     

    PTHV EXTENDED PAY REQUEST (Training & Home Visits):  èßäapp¹ÙÍø PAT-represented educators are eligible for extended pay for PTHV training and home visits if they occur before or after your regular work day.  Your eligibility to receive extended pay will be verified either through a PTHV training attendance roster or a Home Visit Reflection entry (a link to the Reflection Form is below).  If eligibility cannot be verified by the submission deadline on the 15th of each month, the extended pay request will roll over into the next pay period.

     

    PTHV HOME VISIT REFLECTION FORM:  Once you are trained in the PTHV home visit method, you may decide how, when and where to conduct your home visits.  A requirement of the model is to complete a home visit reflection form for each home visit; the insights you gain from this exercise are instrumental in building your capacity to create a welcoming and culturally affirming classroom.  A submitted reflection form is required to verify your extended pay request.
