    ALL Roosevelt students must have this form on file in order to attend the 2020 Winter Formal dance. 

    Forms can be returned by Wednesday, Jan. 29th at 3:30 p.m. to the Activities Director (Mrs. Tamez) in Rm. 292 or emailed to ntamez@pps.net by 3:30 p.m. on Wednesday Jan.29th, 2020.

    In order for a non-RHS student to attend an RHS all student body hosted event (dances, carnivals, etc.), the following guest registration form must be completed and turned in to the Activities Director by the form deadline. Read the form carefully, gather all appropriate signatures, and return promptly to guarantee approval of attending the event.
